Eligibility: Patients aged 12 and younger without periodontal disease.
Annual Fee: $249 per child (optionally payable at ~$23/month)

2 Professional cleanings
2 Exams (comprehensive or periodic)
Routine dental X‑rays (as needed)
2 Fluoride treatments
1 Emergency exam and X‑ray (if needed)

Sealants & additional fluoride treatments – 20%
Simple fillings & simple extractions – 15%
Crowns, bridges & veneers – 10%
Root canals & major fillings – 10%
Orthodontic retainers – 10%
Membership Terms
This plan does not cover periodontal maintenance or major restorative procedures typically needed for adults.
Cannot be combined with dental insurance or other discounts.
Membership is non‑refundable and valid for 12 months from enrollment.
